This Privacy Policy ("Policy") explains how MedHOK, Inc. ("MHK") collects, uses, and discloses information obtained through the Cares Mobile Application for iOS and Android (the "Application").
The purpose of the Application is to provide you with an easy-to-use platform that allows you to access health information and to communicate and interact directly with your health plan and healthcare providers, including through video calls and other messaging. You may be able to request that the Application send you notifications about upcoming appointments and medication reminders.
By using the Application, you accept this Privacy Policy, and your use of the Application is subject to our Terms of Use, which are available here.

MHK may process your information as directed by and on behalf of your health plan and pursuant to the terms and conditions set forth in MHK’s contractual agreement(s) with your health plan. MHK does not sell or license any information you may provide us through your use of the Application.
To use the Application, you will be asked to select your health plan from a list of plans that license and support MHK’s Cares Application. And you must use the login credentials supplied by your health plan.
Your healthcare provider or health plan may collect, store, process, use, and disclose certain information regarding your use of the Application, including data regarding the characteristics or usage of your device, system, and application software, and peripherals as well as the information you provide or agree to provide to your healthcare provider or health plan. You can permit the Application to access your camera or microphone to support video and audio communications with your health care provider or health plan.
Your use of the Application is subject to your health plan’s notice of privacy practices and terms as well as the notice of privacy practices and terms of healthcare providers you may interact with via the Application. If you have questions regarding your health plan or healthcare providers’ policies or terms, please contact them directly.
MHK does not currently take steps to respond to browsers’ "Do Not Track" signals as no uniform standard to respond to such signals has been developed at this time.
MHK understands the importance of protecting your information. We maintain reasonable technical, administrative, and physical safeguards to protect the confidentiality, integrity, and availability of your information. However, no method of transmission over the internet, or method of electronic storage, is 100% secure. Therefore, while we make reasonable efforts to protect your personal information, we cannot guarantee its absolute security.
The Application is not directed to or intended for children under 13 years of age. We do not knowingly collect personal information from anyone under 13. If you become aware that your child or any child under your care has provided us with information, please contact us.
MHK may update this Privacy Policy from time to time to reflect changes in the law, our data processing practices, or the features of our business.
We will publish the updated version on the Site and indicate at the top of the Policy when it was most recently updated. Your use of the Site will be governed by the current version of this Policy.
